Sign and complete this form to authorize Estafeta USA, Inc. to keep your credit card information on file for ongoing basis. By signing this form you authorize un-scheduled charges to your Visa, MasterCard, American Express or Discover card, which are to be withdrawn automatically. You will be charged for the total amount due upon completion of services rendered. A receipt will be emailed to you and the charge will appear on your credit card statement. Advance notification will be provided for charge amount on services. Transaction will be processed once we receive approval of said amount or if the account is past due 30 days. Agreement is valid until a written notification is given to remove credit card information from file or once credit card has expired.
